Four Programs in One:

1. Categorical Medicine Track (6 PGY-1 positions): This track is best suited for physicians interested in broad-spectrum private practice, fellowship preparation, or an academic career. About half of these residents proceed into fellowships.
   
   
2. Primary Care Track (6 PGY-1 positions): This 3-year program emphasizes the breadth and depth of primary care internal medicine, and is best suited for those physicians interested in broad-spectrum private practice in an urban, suburban or rural setting. Additional experience is available in management of pediatric and adolescent patients, sports medicine, office gynecology, rural health, and common outpatient procedures. Interested residents may also proceed into fellowship from this track, particulary for those fields with an outpatient focus.
   
   Please Note: we recommend applicants rank both Categorical and Primary Care tracks for the purposes of the match. We commonly have residents change tracks so please do not let the flexibility of our training opportunities limit your chances in the match! You do not have to specifcally apply to both Categorical and Prelimary in ERAS in order to rank both programs.
   
   
3. Preliminary Medicine (6 PGY-1 positions): This 1-year program is designed to provide a broad survey of topics in adult medicine. It is designed for those needing an internship prior to training in anesthesiology, ophthalmology, dermatology, neurology, radiology and PMNR, for those going into the military, and for students who are undecided about their career plans.
   
   
4. Combined Internal Medicine / Family Practice (2 PGY-1 positions) - This new program combines the best elements of internal medicine and family practice training into a 4-year program. It is designed for those residents seeking both breadth and depth in both disciplines, but with the flexibility to customize the curriculum to meet their individual training goals. Hospitalist Pathway: This 2-year pathway is available at the PGY-2 level, and prepares residents for careers in hospital-based medicine. While continuing to follow the categorical curriculum, there is increased emphasis on hospital-based electives and selectives. Unique ward and intensive care opportunities are available, as well as procedural experience and added focus on cost-effective medicine and clinical care pathways. Specific experiences include:

• Hospitalist preceptorship. This rotation is a one-on-one interaction between the resident and one of the members of the Private Hospitalist Service (and can alternatively be completed at St Vincent Carmel hospital). Topics specific to hospital medicine during this rotation include coding & billing, quality improvement, resource utilization, protocol development, and advanced clinical topics related to inpatient care.
  
  
• Consultative and perioperative medicine. Residents may spend one month during their 2nd or third year with the hospitalist and interns assigned to the consultative service. The focus will be on perioperative medicine and will stress the co-management model of care. Specific topics to be covered include the pre-operative evaluation, perioperative diabetes and steroid management, prevention of post-operative DVT, and management of post-operative fever.
  
  
• Inpatient Geriatric / Palliative Care rotation. Residents may spend one month during either their second or third year on the inpatient geriatric and / or palliative medicine service. Examples of topics which could be covered include end-of-life decision making, pain management and palliative care, communication and determining decision-making capacity.
  
  
• Hospital-based research. During their residency, residents in the Hospitalist track will be required to complete one research project focused on hospital-based clinical research or quality improvement.
  
  
• Other elective rotation opportunities including Maternofetal medicine; Rural Hospitalist Medicine; Administrative Medicine / Quality and Safety; Procedural Medicine (typically involving a combination of Hospitalists and Corvasc Interventional Radiology).
  
  
• Additionally, residents in the Hospitalist track are encouraged to attend the State and National Society for Hospital Medicine meetings and the St.Vincent Health Physician Leadership series.

Women's Health Pathway: This 2-year pathway prepares residents to practice general internal medicine with an emphasis on the care of women. Unique experiences are provided in outpatient, inpatient and clinic settings that cover primary, subspecialty and preventive care from adolescence through geriatrics. Beginning in PGY2, the resident begins one half day per week of outpatient Gynecology clinic, in addition to the Internal Medicine longitudinal ambulatory training.

Specific Women's Health rotation opportunities include:

• Adolescent medicine, sports medicine, geriatric medicine
• Gynecology, gynecologic oncology and urogynecology
• Maternofetal medicine
• Reproductive endocrinology, family planning, genetic counseling, breast health
• Preventive/women’s cardiology and treadmills
• Community medicine in IM women’s clinic
• Endocrinology (osteoporosis/menopause) and alternative medicine
• Psychiatry
• Colposcopy and endometrial biopsy procedure workshop
• 3rd world international health elective
• Volunteer opportunities at Women’s shelters, HIV coordination center, other clinics

Fellowships: The Department of Medicine also supports fellowships in geriatrics, general and interventional cardiology and cardiac electrophysiology, and advanced dermatology.
